How to Apply for Birth Registration Online

  • You need to download or Get a Registration Form from the registrar’s office (from your municipal authority of your state). These days its done by hospital staff.
  • If the child is take born in a hospital, the form is provided by the Medical Officer In-charge.
  • You need to Fill in the form within 21 days of child’s birth
