Procedure to change the name in the passport in India

1. To complete the first stage, a name change affidavit must be completed, detailing the applicant’s information for the old name, new name, the purpose for the name change, and the address. The new name will be announced in the Gazette once the procedure is finished, and copies must be kept for future use.

2. Publication of newspaper ads: Applicants need to publish their names in the local English newspaper and another in their mother tongue language newspaper.

3. Contact the passport service centre to request a new passport: You have two options for submitting the application: online or in person at the centre. You have to download Form No. II for Miscellaneous Services from the website and have to submit it with the form.

• Any individual who wants to do such changes in their passport must apply for re-issuing of passport. For re-issuing the passport, they can visit the Passport Seva Kendra, or either they can apply online by logging into the official website of Passport https://portal2.passportindia.gov.in/AppOnlineProject/welcomeLink#.

• You have to download Form No. II for Miscellaneous Services from the website and have to submit it with the form.

• You have to submit your original old passport with the self-attested copy of the first and last page of the passport.

• Married females who want to change their name after the marriage, have to submit a copy of their spouse’s passport along with the attested copy of their Marriage Certificate or any evidence that proves their marriage like photos. They can also submit an equivalent affidavit as per Annexure D.

• Divorcee Females must submit a certified and attested copy of the Decree of Divorcee, an affidavit, or a Deed Poll can also be submitted as per Annexure E.

• In a case where a female wants to change her surname after the remarriage, they have to submit the attested copy of the Decree of Divorce, and in the case of the spouse’s death, an attested copy of the Death Certificate. They also have to submit proof of her second married i.e., a Marriage Certificate or Photos as evidence.

• If someone wants to change their surname or middle name except for the reasons mentioned above, then they have to submit a Deed Poll or an Affidavit as per Annexure E with the original advertisement in the Newspaper, One English and One Regional, announcing that you are changing your name.
• Your request will be accepted after 30 days of publishing the advertisement.

Documents required for a government employ to change their name in Indian passport

The following documents are required for a name change in Indian passport:

1. A Prior Intimation Letter is necessary for the employee to inform the governing body 
2. Administrators and authorities that object should take their concerns directly to the RPO.
3. There won’t be any modifications to the passport application process from what is required of regular applicants.
4. Relatives of employees must follow the usual passport application process.

• To apply online, applicants must register with Passport Seva at https://portal1.passportindia.gov.in.
• Then they can fill out and submit an application.
•  Following the fully completed application, they can proceed to the payment.
•  People can make an appointment, once the payment is approved.
• The applicants need to appear at the Passport Office On the scheduled day.
• The applicants can then get a token, present all of their paperwork, and supply any necessary biometric data and photos.
• The candidates can then receive tokens, present all of their documentation, and provide any required biometric information and pictures. 
• After the verification process is finished, the passport office will give the applicant a receipt.
• It’s not necessary to determine whether the documents you provide are necessary for the PPC or not.
• You’ll receive a courier to deliver the passport to your home.
