- Married females who want to change their name after the marriage, have to submit a copy of their spouse’s passport along with the attested copy of their Marriage Certificate or any evidence that proves their marriage like photos. They can also submit an equivalent affidavit as per Annexure D.
- Divorcee Females must submit a certified and attested copy of the Decree of Divorcee, an affidavit, or a Deed Poll can also be submitted as per Annexure E.
- In a case where a female wants to change her surname after the remarriage, they have to submit the attested copy of the Decree of Divorce, and in the case of the spouse’s death, an attested copy of the Death Certificate. They also have to submit proof of her second married i.e., a Marriage Certificate or Photos as evidence.
- If someone wants to change their surname or middle name except for the reasons mentioned above, then they have to submit a Deed Poll or an Affidavit as per Annexure E with the original advertisement in the Newspaper, One English and One Regional, announcing that you are changing your name.
- Your request will be accepted after 30 days of publishing the advertisement.
