Change your name in the central gazette
The applicant has to publish a gazette for the name change in the passport affidavit. The affidavit shall contain the old name of the applicant, the new name of the applicant, and also the reason for the change of name by the applicant. The affidavit must contain details like the father’s or husband’s name and address etc. once the new name of the applicant is published, the gazette copies must be kept for future use.

Procedure to Apply for changing the name in passport, west Bengal
To get an e-form for, the following steps should be taken,
- For a re-issue of a passport or for applying for a fresh passport, the applicant has to register themselves on the website of passport seva online portal.
- After getting registered, the applicant has to log in to their account by using the Id and password.
- From the online portal, the applicant has to download the e-form available, to apply for a re-issue of the passport.
- Fill out the details in the e-form recently and then click on the option of validate and save button. It will generate an XML file. This XML file will be uploaded later in the process.
- The XML file should be uploaded via the option of uploading an e-form. The file must be in XML file form; no other form of a document, such as PDF, will be accepted in the online system.
- After the form is uploaded, the applicant has to pay the fee for the service and book an appointment at Passport Seva Kendra. Now, click on the button “pay and schedule appointment” link, this will generate for the applicant to schedule an appointment with the passport seva Kendra.
- Now, search the location of passport seva Kendra and select the nearest one.
- After the appointment is booked at the passport seva Kendra selected by the applicant, they are required to pay online. The payment can be made by using a credit card or debit card (Visa and MasterCard), internet banking (State Bank of India and only associate banks), or SBI challan.
- There is an online fee calculator available for the applicant in order to calculate the fees for the services rendered to them.
- Application receipt which contains the ARN (Application Reference Number) or number for appointment, the applicant has to take a printout of it.
- Now for the last step, the applicant has to visit the passport seva Kendra where they booked the appointment. The applicant is required to carry all the documents and as well as the application receipt form.
Minors’ documents are required in order to change name on passport
